If you are registering more than one youth, please fill out a separate form for each. You can still make one combined online payment.

Registration will not be complete until payment is made.

GLOW or GO Liability Waiver Form

You’ll receive a liability waiver in your confirmation email. Please click the link to read it, fill it out, and sign electronically. This must be completed before the event.

A separate waiver is required for each youth.
